The role involves supporting individuals in community settings who are experiencing Severe Mental Illness to live fulfilled lives, enable them to stay well and access community support as needed. This will be in collaboration with local Community Mental Health Teams (CMHTs) and other professionals. You will work closely with CMHTs to support recovery-focused care and enable individuals to access the help they need.
Role Description
This role will appeal to people who have:
- Experience of working with children, young people or adults in a health, social care or any other relevant setting
- Have a good knowledge of services across Bradford District and Craven
- A passion to help improve people’s lives
- Have excellent communication skills and feel confident in supporting people in a clear, meaningful and non-judgmental way
- Are confident in recognizing and responding to risk and safeguarding concerns
